We partner with providers to effectively address patient Mental Health care through the Collaborative Care Model. It is the only Integrated Behavioral Health Care model recognized by CMS that is reimbursable for primary providers.

Brooke leverages her 16+ years of leadership experience in the healthcare space, building highly productive teams, driving results, and exceeding business objectives daily. She is a graduate of the University of Central Florida, where she graduated with a Marketing and Public Relations degree.
Chief Executive Officer
Dr. Jordana Hollen is Board Certified by The American Board of Psychiatry and Neurology, and specializes in providing outpatient Psychiatric care for adults. Dr. Hollen earned her B.S. in Psychology from the University of North Carolina at Chapel Hill and completed her Psychiatry residency training at the University of Florida.

Dr. D’Agostino is a Medical Director at Anthem, Beacon Health Options. Previously he was faculty at Weill Cornell Medicine and New York Presbyterian Hospital. He attended Brown Medical School, completed residency at New York Presbyterian/ Weill Cornell, and is an alumnus of Harvard Business School. He is Board Certified in psychiatry and addiction.

As a results-driven exec with progressive experience in H.R., recruitment, and operations support, Heather has a serious talent for H.R. and Admin team development. She has a B.S. degree from the University of North Florida, is certified with SHRM and HRCI, and is a Board Member of the Orlando Muscular Dystrophy Association.
Chief People Officer
Ryan is a leader with 20 years of experience in various healthcare settings ranging from Pharmacist to Regional Vice President. He recently worked at Genoa Healthcare as Regional V.P. of Ops, collaborating with community mental health centers. Ryan received his B.S. in Pharmacy degree from the St. Louis College of Pharmacy.
VP of Operations
Tony’s expertise in IT spans 20+ years, including 15+ years of information security and leadership experience. Key areas include the protection and security of Personally Identifiable Information (PII). Along with being CISSP and ITIL v3 certified, Tony has a MS in Information Systems Management from Virginia Commonwealth University.
VP of Customer Experience
With a history of providing compassionate, holistic care to maximize health outcomes, Kristin uses collaboration with all healthcare providers to offer great care. Kristin has her B.S. in Nursing from Hawaii Pacific University, M.S. in Nursing as an Adult N.P., and Post Grad. Certificate in Psychiatric Mental Health N.P. from Duke University.
Throughout his career, Dr. Dennis has worked in various clinical settings, including the emergency room, long term care, and telehealth. A recognized leader in psychiatry, Dr. Dennis is a Fellow of the APA and completed his residency through the Albert Einstein College of Medicine. He is Board Certified in Psychiatry.
